It shall be the duty of every person operating or driving a vehicle involved in an accident resulting in an injury or death to a dog, cat or other animal to report the same immediately to the Police Department by telephone, and to report the same in writing within 24 hours after the occurrence of such accident to the Police Department, giving the relevant information concerning the accident, the report to be made on forms provided by the Police Department.
